- Indicate the location (ie) Lot/Block & Sunnyside Beach
- The applicant’s name & address & best contact #
- The proposed development: Example: New Residence / New Addition / Replace or Build Deck
- Attach a site sketch: A hand-drawn or professional drawing of an aerial view of your lot showing size of lot & visual placement of all existing buildings including your proposed new residence or addition or deck. The drawing must indicate the number of feet from: the new residence, addition or deck, to all sides of the lot to ensure zoning requirements are met.

Fee $0.55/ square foot- Residence/ Addition
Fee$0.30/ square foot- Deck
- Fill in all owner information as required
- Fill in information Re: Building specs (Foundation/heating/roofing, etc)
- Fill in information Re: Square footage & estimated value of construction
- Attach blueprints
- Date & sign form
- Provide our office with completed Contractor List and attach to Building Permit Application
